我が国では、精神保健医療福祉施策により、精神障害者の地域移行支援が推し進められている。しかし、長期入院をしている重度かつ慢性の症状を有する精神障害者（重度・慢性精神障害者）への地域移行支援は一向に進んでいない。精神科看護は、未だ問題解決型思考を基盤とした看護が主流であり、重度・慢性精神障害者の院内での不適応行動に着目し、対応することが多い。しかし、不適応行動は繰り返され、対応によっては不適応行動が増加することもあり、看護の方向性や適切な対応を見いだせずに苦慮している。よって本研究は、適応行動に焦点をあて、重度・慢性精神障害者の適応行動を維持・拡大するための「研修プログラム」「看護計画」「事例検討会」「カンファレンス」を導入した新たな看護介入モデル『重度・慢性精神障害者の適応行動を維持・拡大するための看護介入モデル』を構築し、その有効性を検証することを目的とする。今後、重度・慢性精神障害者の適応行動を維持・拡大する研修プログラムの開発し、重度・慢性精神障害者の適応行動を維持・拡大するための「看護計画」「事例検討会」「カンファレンス」「看護計画」の実施内容と方法を検討する。これらの研究成果をもとに、重度・慢性精神障害者の適応行動を維持・拡大するための看護介入モデルを構築と有効性の検証をしていく。今年度は、重度・慢性精神障害者の適応行動を維持・拡大する研修プログラムの開発に向け、調査対象となる精神科病院の調整、ならびに研究の概要説明や協力依頼を行った。
