This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. The specific aim of the study will be to compare a cohort of elderly HIV positive females and males with matched HIV-negative controls to determine differences in bone mineral density and biochemical markers of bone turnover. HIV infection is a major worldwide public health problem. Patients in the United States have access to highly active antiretroviral therapy (HAART) that has, in recent years, decreased morbidity and prolonged life. However, these agents are associated with significant metabolic disturbances such as glucose intolerance, hyperlipidemia and lipodystrophy. Initial clinical and laboratory data suggest that patients with HIV with or without HAART therapy are at increased risk of osteoporosis. In parallel, there has been a notable recent increase in the mean age of HIV pateints in whom metabolic disturbances, including possible effects on bone, remain uncharacterized. Therefore, the long-term goal of this study will be to examine the effects of HIV on bone mass in the elderly population. The central hypothesis is that HIV positive elderly women and men have lower bone mass than matched HIV negative controls.
