项目摘要

本研究は、コスト情報を加味したタスク・シフティング（業務移管）関連指標を包摂した病院バランスト・スコアカード（以下、病院BSC）の有用性を検証するために、日仏米の国際比較の観点から整理することで、医療におけるマネジメント・コントロール及び原価計算のあり方を検証することを目的としている。具体的には、病院BSCのKPI指標として、「時間」を配賦基準とした病院原価計算（時間ベースの病院原価計算）で算定されるキャパシティ情報（業務遂行に用いられる資源の量）を用いたタスク・シフティング（以下、TS）の有用性を検証することを目的としている。本研究は、厚生労働省の「医師の働き方改革の推進に関する検討会」で議論されているTSの影響を、コスト情報の観点から院内情報システム（EF統合ファイル・クリニカルパスなど）を活用して検証しようとしている点に、学術的な特色・独創的な点がある。そこで本研究では、①病院BSCにおけるTS関連情報の有用性を明らかにすることと、②院内情報システムを活用した病院BSCおよび病院原価計算（特にTDABC）の設定運用プロセスを明らかにすること、これら2つの研究課題に取り組むことで、コスト情報を加味したTS関連指標を包摂した病院BSCの体系的な分析整理を行う。以上の研究を行うにあたり、TSと病院BSCの関連性について研究調査を進めていきながら、同時に時間主導型活動基準原価計算（TDABC）の研究成果をTSに関連させながら研究を進めていく。具体的には、これまで集めてきたTSの基本的文献・資料のほか、最新の病院BSCや病院TDABCの事例を掲載した書籍や学会誌などの資料収集も行う。文献収集だけではなく、国内を中心に実際の病院訪問調査を行い、病院BSCおよび病院原価計算に対する現場の意見も積極的に取り入れる。

The purpose of this study is to examine the usefulness of hospital related indicators, including hospital related indicators (hereinafter referred to as hospital BSC), for international comparison of medical information, medical information and original calculation. Specifically, the KPI index of the hospital BSC is assigned to the "time", and the hospital origin calculation (the hospital origin calculation of the time) is calculated. The information (the amount of resources used in the operation) is used to verify the usefulness of the application (TS). This study discusses the impact of TS on the promotion of physician's prescription reform in the Ministry of Health and Welfare, and discusses the points of information integration and hospital information integration (EF integration). This study includes the following aspects: (1) the usefulness of TS-related information in hospital BSC;(2) the setting and application of hospital information system in hospital BSC and hospital original calculation (especially TDABC);(3) the selection of TS-related information in hospital BSC system; and (4) the analysis and arrangement of TS-related information system in hospital BSC system. The above research is conducted in the context of TS and hospital BSC correlation, and the results of time-based activity base computing (TDABC) research are conducted in the context of TS and BSC correlation. Specific information on the collection of basic literature and data, the latest cases of BSC and TDABC in hospitals, and the collection of data in the journal of the Library Society The literature collection includes a survey of international hospital visits in China, a survey of on-site comments on hospital BSC and hospital original calculations, and a survey of on-site comments on hospital evaluation.
