项目摘要

本年度は、多職種連携協働、コラボレーティブ・リーダーシップ、組織学習、医療安全等に関する調査を実施してデータを取得した。A病院を対象とする研究では、AITCS-Ⅱ-J（Assessment of Inter-professional Team Collaboration Scale-Ⅱ-J　AITCS日本語版）を用いて、多職種連携の実態と医療の効率性を示す指標として病床回転率，平均在院日数，ADL改善率との関係を調査した。AITCS-Ⅱ-Jのサブスケールであるパートナーシップ得点が高いほど，医療の効率性指標である病床回転率が上がり，かつ平均在院日数が有意に短縮されることが明らかになった。具体的には，患者の要望に耳を傾ける行動，ケアプラン作成には患者や家族と一緒になって行う行動を含む多職種連携を推進、醸成することにより平均在院日数が短縮することが示唆された。B病院との共同研究では、組織学習を説明変数、多職種連携協働（AITCS-Ⅱ-J）を目的変数、コラボレーティブ・リーダーシップ（AICLS日本語版）を媒介変数とする共分散構造分析を行った。組織学習とコラボレーティブ・リーダーシップの関連は、β＝0.80（ｐ＜0.001）、組織学習と多職種連携協働の関連は、β＝0.37（ｐ＜0.001）、コラボレーティブ・リーダーシップと多職種連携協働の関連は、β＝0.58（ｐ＜0.001）であった。媒介を仮定したモデルでは、コラボレーティブ・リーダーシップが、組織学習と多職種連携協働の関係に及ぼす間接効果は0.47であり、コラボレーティブ・リーダーシップが組織が学習と多職種連携協働に影響を与え、媒介することが示唆された。カナダ、オーストラリア、ドイツ、ルワンダ等の国際共同研修者と連携し、各国の多職種連携の実態に関する実証研究成果をSpringerNatureよりInnovative Collaboration in Healthcareを2023年に刊行する予定である。
