项目摘要

本研究の目的は、終末期を迎えた認知症患者の意思を、介護者たちがどのように読み取り、解釈しているのか、特に介護家族の役割に注目しながら明らかにすることだ。認知症介護においては現在、患者本人の意思の尊重が重要な目標として掲げられている。しかし、認知症が記憶障害やコミュニケーションの障害を特徴とする以上、様ざまな困難が生じる。そこで介護家族はしばしば、患者のライフヒストリーに基づいて患者本人の意思を解釈し、選択を代行しようとするが、それでも限界が生じる。そうした困難を先鋭化させたのが、2020年から続くコロナ禍である。2022年度は、これまで長年調査に協力してくれていた介護家族Iが実母Kを、同じくMが実母Lを、それぞれコロナ禍の最中に看取った。彼らはそれぞれ、認知症患者を看取る上で、非常に特徴的な経験をしている。しかし、看取り直後の介護家族に対し、本格的な調査を実施することには倫理上の問題が考えられる。そこで2022年度は、あくまで予備的な調査に留め、本格的な調査は2023年度に実施することとした。また2022年度は、複数の家族会（介護家族の互助グループ）と連絡を取り、2023年度に本格的な調査を実施できるよう、打ち合わせをした。いずれの会も長年の調査協力団体だが、コロナ禍でこれまで、本格的調査が難しい状態にあった。今回の研究目的に即した調査を実施するべく、スケジュールを調整している。さらに2022年度は、これまでに集めたデータをもとに、「認知症の人の心はどこにあるのか」（『三田評論』1271号）、「変わる家族介護の姿」（『現代思想』50(14)号）という2つの論文を発表した。前者は、介護家族が認知症の人の「こころ」を読み取る相互行為場面を分析したものであり、後者は、家族介護の歴史的な変遷を分析したものだ。2023年度も、調査を発展させつつ、並行してデータを分析し発表したい。
