项目摘要

≪目的≫近年、高齢者施設における看取りが増加傾向にある。申請研究では、社会的学習理論に基づき、高齢者施設の介護職員の看取りケア行動を促進する要因として、「看取りケア効力感」を新たに概念化した。そして、看取りケア効力感の測定方法と関連要因を検討した。更に、看取りケア効力感と親和性の高い概念であるpreparedness for death(看取りに対する準備性)に着目し、両概念の関連性と先行研究の結果を整理することで、看取りケア効力感概念の理論的拡大を行う。それらの結果を踏まえ、最終的に、看取りケア効力感を維持する教育的介入方法を検討することを目的とする。≪本年度の研究≫まず、preparedness for death（看取りに対する準備性）の先行研究のレビューを行い、看取りケア効力感との関連を理論的に整理した。そして、１）看取りケア効力感尺度の再検討、２）看取りケア効力感と看取りに対する準備性との関係性の検討、３）各概念の先行研究において関連が検討された看取りケア経験、悲嘆反応などについて、両概念との関連を探索的に検討することを目的として、インターネット調査を実施した。高齢者施設で介護職として勤務経験のある者を対象として調査を実施した結果、579名(男性220名、女性359名)から回答を得た。今後、目的を踏まえ詳細に分析する予定である。また、これまでの研究で、看取りケア効力感を維持するための要因として達成経験が重要であることが明らかになっている。そこで、達成経験を積むための方法として「遺族からの評価」と「自己評価」に着目し、申請期間を通して、大阪府の特別養護老人ホームにて遺族アンケート及び介護職員による評価シート記入の実践を行ってきた。今後、これらの実践内容について、看取りケア効力感を高めるための教育的介入方法として、更に改善を図る予定である。
