本研究は、高齢者の事前指示書（ADs）の作成要件を家族機能、曖昧性耐性、終末期医療の希望の3点から明らかにし、ADs作成を支援する個別介入プログラムを構築することであった。平成30年度には全66問の自記式質問紙を作成した。平成31年度には、中部地方および近畿地方の高齢者団体および施設計4か所において、65歳以上の男女216名が本研究に参加され質問紙に回答された。研究参加者の年齢は65~70歳が71名（32.9%）で最も多かった。ADs書面化は28名（13%）で実施済みであり、118名（54.6%）は未実施であった。57名（26.4%）はADsを知らなかった。家族機能は「中間群」が115名（53.2%）であった。曖昧性耐性は、「曖昧さの享受」の得点が高かった（Avg. 26.35, SD 4.64）。ADsの考えに賛成し、作成意思を持つ方が7割以上を占めた。「曖昧さの享受」および「曖昧さの統制」と、ADsの考えに賛成し作成意思を持つことに関連がみられた（CV. 0.28~0.37）。家族機能は「曖昧さへの不安」「曖昧さへの受容」に対し弱い関連があった（CV. 0.11~0.17）。特にバランス群は、ADsの考えに賛成し作成意思を持つことに関連があった（CV. 0.14~0.15）。以上から、高齢者のADs作成支援は家族機能が中間群を主な対象とし、まずはADsを周知することが必要である。終末期医療についての情報が不十分なことが不安要素になるが、それに親和性や寛容性も併せ持つため、書面化しない傾向にある。よって、ADs作成支援では、対象者にポジティブな態度でADs作成に関連した曖昧な内容を明らかにした上で、終末期医療および事前指示の実際についての情報提供を継続的に実施し、自己対処力を支援する体制作りが重要であることが明らかとなった。

This study focuses on the three aspects of family function, ambiguity tolerance, and end-of-life medical hope, and the construction of individual intervention programs to support the creation of advanced medical instructions (ADs).
