项目摘要

本研究の目的は、悉皆性の高いリアルワールドデータである電子カルテデータや全国レセプトデータを用いて、国内外のポリファーマシーの実態を定量的に評価し、多剤併用に関する医療政策介入と臨床側の処方変容の相関を総合評価する手法の確立案を検討することである。今年度の活動として、ポリファーマシーとなる薬剤の種類として主要オピオイド鎮痛薬の使用量の状況について明らかにし、がん患者を特定として医療用麻薬の使用量とポリファーマシーの関係性について横断的に解析した。さらには、小児精神疾患に処方される向精神薬の処方量の推移を経時的に調査した。全国レセプト情報・特定健診等情報データベースに基づいて、入院オピオイド処方量と外来オピオイド処方量の比を全国・年齢ごとに算出した結果、硫酸モルヒネ、フェンタニル、オキシコドンなどによる処方量は、調査期間中に院外処方で増加傾向を示した。また、人口統計学的変数の中で65歳以上の患者は、オピオイド種類および処方量の年間漸減傾向と有意に関連していることを明らかにした。さらに、小児精神疾患における3種類以上の向精神薬の処方量は院内処方において減少実態を明らかにした。オピオイド系鎮痛薬はがん患者など緩和医療の向上に貢献することもある。ポリファーマシーの患者にとっても、加齢による生理的な変化や複数の併存疾患を治療するために疾患や症状に合わせた適切な薬物療法が行われることが重要である。このような医薬品が持つリスク・ベネフィットの両面を考慮したポリファーマシーによるオピオイド鎮痛剤の相互作用など、今後、国際比較を含むさらなる検討が必要である。
