【研究目的】分子標的薬剤など新規抗がん剤の登場は、治療効果の向上と生存期間の延長をもたらしたが、患者の経済的負担を増加させ、病院経営を圧迫しているともいわれる。本研究は、電子カルテ記録をもとにがん薬物療法における薬剤使用評価(Drug Use Evaluation)を実施し、得られた情報を安全性向上と医療費軽減に向けて活用することを目的とした。【研究方法】電子カルテ・データの2次利用に特化したデータベースOpen Data Base(ODB)を用いて、開発環境としてCyber Frameworkを利用して、注射薬オーダー検索機能及び払い出し本数集計機能を有するアプリケーションを申請者自身が作成した。次に作成したアプリケーションを利用して、処方頻度及び医薬品購入金額の上位を占める抗がん剤を対象として2003年3月から2007年5月を調査期間とし、以下の項目について検討した。・外来及び入院における抗がん剤の処方動向及び使用量の推移・後発医薬品の抗がん剤採用における経済的効果【研究成果】電子カルテの注射薬オーダー記録を薬剤名、投与期間、診療科、実施場所を検索条件とし、結果には投与日、患者名、薬剤名、投与量、診療科、実施場所、実施状況などを一覧表示し、薬品別払い出し数の集計機能を有する患者横断的に検索可能なアプリケーションを開発した。アプリケーションを利用した使用状況調査の結果、調査した抗がん剤の多くは、外来での使用が大半を占め、本院での抗がん剤使用量は年々増加傾向にあり、外来における抗がん剤の使用量は入院と比較し大きく増加していた。後発医薬品の採用により、抗がん剤の医薬品購入額及び患者あたりの薬剤費は先発品利用時と比較し抑制できることが明らかとなった。この成果は、第17回日本医療薬学会年会において、"がん化学療法における医療経済学的視点に基づく薬剤使用評価"というタイトルで公表した。
