国内の医療費は2010年度で36.6兆円、伸び率も対前年度比3.9%増となり、医療費の抑制は国家予算および愚者負担の面からも重要な課題の一つである。この中でがん治療にかかる治療費は他の領域の疾患の治療費に比べて高く、全体の10%以上を占める。新規の抗がん剤の開発や化学療法レジメンの確立は、治療成績の向上、副作用の軽減、外来通院治療への移行など多くのメリットを有するが、その一方で、高額な薬剤費や長期に渡る抗がん剤治療の継続により、患者に多くの経済的負担を強いているが現状である。これらの問題に対して、医療現場サイドで行える対策の1つとして、抗がん薬の複数規格の採用が挙げられる。抗がん薬を複数規格採用し、その内から最安値となる規格の組み合わせを考えることで薬剤費を大幅に軽減することが可能となる。しかし、この組み合わせを考える際には、抗がん薬の指示量に応じて薬価や各規格の使用量を踏まえて検討するため数分の時間がかかり、処方する医師や処方鑑査を行う薬剤師にとって無視できないタイムロスとなる。この解決策として、Microsoft Excel^[○!R]のプログラム作成機能を利用して、複数規格を持つ抗がん薬からどのような指示量に対してもコストが最安になる組み合わせを瞬時に導き出すツールを開発した。本ツールはExcel上で動作するため高額な専用プラグラムの導入を必要としない上、Windows、Macの両OSで動作する。また、使用方法も、採用品目を選択し処方量を入力するのみという、簡便な操作である。このツールを用いる事でコスト計算にかける時間を大幅に短縮できることから、自施設内においてその有用性は高く評価された。今後、当院薬剤部のHP等を介して他施設でも自由に使えるものとする予定である。
