1980年から1994年までの15年間に当科にて一次治療を行ない,かつ治療経過が検索可能であった上咽頭癌症例73例を対象とした。73例の5年生存率は58.1%,10年生存率は48.8%であった。ルナ染色にて好酸球浸潤が認められたのは40例で,全体の54.8%であった。その内訳はTATE(Tumor Associated Tissue Eosinophilia)grade1:20例,grade2:9例,grade3:7例,grade4:4例であった。好酸球浸潤のみられた40例の5年生存率は73.5%であり,浸潤のみられなかった33例の46.6%と比較し,良好であった。in situ hybridizationにてMMP(Matrix metalloproteinase)-1 mRNAの発現が認められたのは21例で全体の28.8%であった。発現は分化度の高い扁平上皮癌に求められる傾向があったが,TNM分類や5年生存率との相関はみられなかった。MMP-1mRNAの発現とTATE gradeの関係を検討するとMMP-1mRNAが認められてのはTATEgrade4で2例(50.0%),TATEgrade3で5例(71.4%),TATEgrade2で5例(55.5%),TATEgrade1で4例(20.0%),TATEgrade0で5例(15.2%)とTATEgradeの高い群に多く認められた。また確定診断より2年以上の経過観察が可能で,かつ局所再発,遠隔転移を認めない症例を予後良好例確定診断より2年以内に局所再発もしくは遠隔転移を認めた症例を予後不良例とすると,TATEgradeが4もしくは3のhigh gradeで,かつMMP-1mRNAの発現が認められた7例はすべて予後良好例であった。以上より上咽頭癌における好酸球浸潤およびMMP-1mRNAの発現は予後因子となる可能性が示唆された。今後は症例数を増やすとともに,in vitro,in vivoでの基礎的検討を行なう予定である。

During the 15-year period in 1980 and 1994, when the department was treated once and for all, 73 cases of cancer of the upper pharynx were diagnosed. The 5-year survival rate of 73 cases was 58.1%, and the 10-year survival rate was 48.8%. There were 40 cases of acid ball immersion, all of which were 54.8% of them. There were 9 cases of TATE (Tumor Associated Tissue Eosinophilia) grade1:20, 9 cases of grade2, 7 cases of grade3 and 4 cases of grade4. The 5-year survival rate of 40 cases was 73.5%, and that of 33 cases was 46.6%. In situ hybridization diagnosis MMP (Matrix metalloproteinase)-1 mRNA showed that there were 21 cases of abnormal symptoms in all patients. The degree of differentiation of flat epithelial carcinoma was high, and the 5-year survival rate of TNM classification was similar to that of normal controls. MMP-1mRNA showed that there were 2 cases of TATEgrade4 (50.0%), 5 cases of TATEgrade3 (71.4%), 5 cases of TATEgrade2 (55.5%), 4 cases of TATEgrade1 (20.0%), 5 cases of TATEgrade0 (15.2%) and 5 cases of TATEgrade (15.2%). Make sure that you have been cut off for more than 2 years. After observing the possibility, the bureau has revisited, and the good case has been confirmed. The case has been confirmed by the Bureau within 2 years. The bad case has been confirmed, and the TATEgrade has 4 cases. 3 cases have been high grade, and 7 cases have been diagnosed by MMP-1mRNA. The above cancer of the upper pharynx indicates the possibility of acid ball immersion, MMP-1mRNA, and the possibility of a future factor. In the future, the number of cases will be affected, and the number of cases of in vitro,in vivo will be confirmed.
