项目摘要

<研究対象者> 北海道内の3つの病院14病棟において17名の看護婦の協力を得てインタビュー調査を行った。この17名は、看護部及び婦長から患者、家族への告知場面で看護婦の役割をとった経験がある者として推薦された者である。対象者の勤務年数は平均12年であったが推薦により対象者を決定したため2から29年までと多様であった。さらに、対象者の挙げた事例にかかわった婦長、同僚看護婦、患者、家族及び医師からも協力を得て、合計42名の関連者達にもインタビューを行った。患者からの協力が得られない場合には看護記録等から情報を得た。<結果>対象者が語った話題における告知の性質は、「治療、手術のリスク」(8名)、「予後不良」(7名)「癌告知」(2名)であった。それらの中心的話題は、「自己決定の促進」(7名)、「病状の正しい認識の必要性」(6名)、「本人に告げるべきか」(4名)の3つに分かれた。17名中16名は、語った事例に対して受け持ち看護婦であったため告知前から告知後、退院まで、あるいは終末を向かえるまでの経過について語ってくれた。対象者は、「告知」という出来事にあえて関わっているのではなく、患者、家族が十分な情報の中で自己決定ができるように、あるいはQOLを重視できるように思考し援助を行っていた。その役割は、患者、家族が自己決定できるための指針提示役、あるいは調整役であった。その援助方法として患者、家族、同僚、医師と積極的に話し合いの時間を持ち、その時には時期やタイミングを重視していた。患者に関する情報を多く持ち、それらを医師、同僚に伝え、話しあうことで最善の方法を思考し、行動することができていた。患者、医師と対立する立場をとるのではなく、支援が必要な立場へと移行する調整役の立場を看護者の立場として打ち立てていた。そして最終的には、患者、家族、医師、同僚看護婦にも影響を与えて、1つの合意、方向性を導いていた。
