【対象】SD雄ラット(体重約600g)【方法】全麻下,頸部に正中切開を置き気管を露出する.気管の2/3周を切離して以下の4種類の方法で吻合処理をする.(1)A群:吸収糸による気管吻合群(n=30).(2)B群:気管吻合部をフィブリン接着剤で被覆(n=30).(3)C群:気管吻合部をフィプリン接着剤とOK-432で被覆(n=30).(4)D群:気管吻合部をフィプリン接着剤と吸収性メッシュで被覆,補強(n=30).【評価法】術後7日目,14日目,21日目に各群10匹を犠牲死させ(1)肉眼的所見,(2)組織学的所見,(3)抗張力,(4)コラーゲン量(ハイドロキシプロリン量)の4項目について比較検討する.【結果】術後7日目,14日目,21日目におけるA群のハイドロキシプロリン量は,それぞれ7.4,6.1,8.5mg/gWetであった.B群のハイドロキシプロリン量はそれぞれ11.0,7.3,8.6mg/gWetであった.さらに組織学的検討ではB群ではA群と比較して,術後7日目において,より多くのコラーゲン組織の発達が観察された.【まとめ】フィブリン接着剤は気管吻合部において術後早期の段階で治癒を促進させる効果を有する可能性が動物実験で示唆された.現在,気管吻合部の術後の経時的な抗張力変化およびOK-432や吸収性メッシュによる被覆,補強効果についても比較検討中である.

[Objective] SD male (weight about 600g)[Method] Under general anesthesia, the neck was opened in the middle and the tube was exposed. 2/3 weeks after the tube is cut, the following 4 methods are used to match the treatment. (1)A Group: suction tube coincidence group (n=30). (2)B Group A: the anastomosis part of the gas tube is covered with a coating (n=30). (3)C Group: tube anastomosis part OK-432 (n=30). (4)D Group A: the anastomosis part of the tube was covered and reinforced (n=30). [Evaluation method] Ten animals in each group were sacrificed on the 7th, 14th, and 21st day after operation.(1) Macroscopic findings,(2) Histological findings,(3) Tensile resistance,(4) Measurement, and (4) Comparative evaluation. Results: At 7, 14, and 21 days after operation, the concentrations of group A and B were 7.4, 6.1, and 8.5 mg/gWet, respectively, and those of group B were 11.0, 7.3, and 8.6 mg/gWet, respectively. Histological examination of group B and group A showed that there was no significant difference between group A and group B. The possibility of promoting healing in the early postoperative stages of the anastomosis is demonstrated in animals. Now, the tension resistance of the anastomotic part of the tube after operation is changed too much. OK-432, the absorption property is changed, the coating is changed, and the reinforcement effect is changed.
