项目摘要

心身障害児とその母親の母子相互作用を促進する看護援助として、対象母子の食事場面をビデオテープの分析・援助への活用を試み、その結果母親が自分の行動や子どもの反応を客観的に捉える機会となり、子どもの発達段階を見据えて関わるようになるなど有効であるという示唆を得たが、実験的な試みであり、今回は臨床で応用するための検討を目的とした。母子相互作用の初期アセスメントとして用いるNursing Child Assessment Satellite Training Feeding Scale及びTeaching Scale(以下、NCASTとする)に関して、平成12年度に使用の許可を受けた。一方で、ビデオの編集技術を洗練するためにビデオ編集システムを購入し、指導用ビデオの編集は容易となった。平成13年度は、肢体不自由児施設において対象に対する看護援助を開始した。まず、臨床の看護スタッフと共に対象となる事例の選択を行い、日常の食事場面のビデオ撮影を依頼した。この際、撮影時のカメラの設置方法や撮影方法のオリエンテーション不足により、予定していたコーディング方法を採用できないものがあった。また、1回の母子入園(8週間)で数組の母子を対象とするのは業務の中では困難であり、期間中に予定していたデータ数を得ることができなかった。対象となった2事例のいずれも、援助開始前と比較してNCASTスコアおよび母親の言動より、母子相互作用が促進されたことが示された。また、研究者の行った初期アセスメントを臨床の看護スタッフと共有することにより、アセスメント結果をケアプランに組み込むことができ、他職種ともケースカンファレンスなどで共有することができるなど、臨床において母子相互作用を促進する看護援助方法・技術を検討していく上で、多くの示唆を得ることができた。今後例数を重ねて検討をしていく予定である。
