项目摘要

本研究は、高齢者福祉における在宅か施設かの二者択一ではない第3の選択肢として「サポ'一トセンター(地域ケアサーピスの拠点)」を構想し、地域ケアの成立要件を検討するものである。構想は、ケア内部完結型の「施設」という枠組みを解体し、施設ケアサーピスを地域に開くことやその逆も想定して、既存のケアサーピス資源を有効活用した24時間の地域ケアにより、高齢者が在宅で住み続けられる仕組みである。こうした仕組みの成立には、制度や経済あるいは高齢者の心理的問題等の他に、高齢者の居住地点とケアサーピスの拠点間の「距離」が問題となり、本研究はこの「距離」に着目した。(1)本年度は昨年度から引き続き、拠点過疎地域における地域福祉環境について分析を行い、下記の結論を得た。拠点過疎地域では、小圏域の二一ズを逸脱した大規模施設の建設が、地域の居宅サーピス利用者を施設に吸引するように移動させ、結果として地域の居宅サーピスが利用者減によって弱体化すると言った構造が明らかとなった。介護内部完結型の施設が居宅サーピスと利用者を取り合うのに比べ、ケア外部利用型(居宅サービス利用)の住形態であれば、地域の居宅サーピスは相乗的に強化される。この意味から「施設の圏域・規模設定」と「居宅サーピスと住形態のパランスのとれた連携」の検討が必要である。(2)また実存する5拠点のサーピス圏域を対象にシミュレーションを行い下記の結果を得た。老年人口の増加によって訪問先が増加する際に、1日のヘルパーの移動距離がどのように変化するかをシミュレートした結果、ヘルパーの訪問箇所数の増加に伴い移動距離は比例的に増加するが、一定数以上となると移動距離がほぽ横ばいとなることなどが明らかになった。地域の老年人口の変化と移動距離の関係性が見えつつあり、発展して在宅を支えるに相応なサーピスを提供する際の、サービス提供圏の大きさ、拠点の地域配置、住形態との連携方法などの検討を行うについての前段階の成果が得られた。
