本研究では、訪問看護を利用する要介護高齢在宅療養者の口腔衛生状態向上に向けた口腔ケアプログラムを開発し、臨床での実施可能性を評価することを目的とした。研究実施計画では、2022年度12月までに先行して行った実態調査の結果の分析や文献レビュー、フォーカスグループインタビューを通し、口腔ケアプログラムを構築し、2023年より調査開始、2023年度内での結果の分析・執筆、投稿までを目指すことを予定していた。2022年度は予定通り12月までに、先行して行った実態調査の結果の分析・文献レビュー、口腔ケアの専門家との意見交換を基に、プログラム案を構築した。次いで、それらの内容を踏まえた研究計画書を、所属機関の倫理審査委員会に提出し、倫理審査の承認を得た。倫理審査の承認後、介入調査を開始した。調査計画に従い、適格基準に沿った施設を選定し、施設管理者に研究説明および研究協力の依頼を行い、研究同意を得た。その後、施設に所属する訪問看護師に対し、研究説明を行い、研究参加の同意を得た。訪問看護師は、本プログラムのファシリテーターとしての役割を担うため、プログラムに沿って、研究者が口腔ケアに関する講義を実施した。所要時間はおよそ３０分とし、誤嚥性肺炎の発生機序、口腔ケアの効果、対象者に合わせた口腔ケアの方法について講義を行った。講義を受講した訪問看護師と共に、サービス提供時に合わせて対象者の自宅を訪問し、研究者より対象者へ研究説明および同意取得を行った。現在、目標症例数の1/4程度の調査を開始しており、年内までのデータ収集を目指している。

The purpose of this study is to evaluate the clinical feasibility of oral hygiene development and the use of visiting care to protect the oral hygiene status of high school inpatients.
