昭和59,60年度に引続き、交通事故の被害者の救急医療体制と医療費問題について研究および実態調査をした。今年度は、公衆衛生の専門家の参加を得て、とくに医療側からの研究に力を入れた。まず、救急医療については、都市と比べて比較的体制の整備のおくれている地方の実態を調査した。宮崎市では、宮崎市の医師会病院に付置された救急医療センター的機構の活動状況と問題点、経費負担について調査した。次に、へき地の問題解明のため、五島列島の福祉における救急体制について実態調査を行った。へき地も含め、徐々に整備されてきてはいるが、救急患者の搬送と、受入れ医療機関の連携が必ずしも円滑にいってはいないため、種々の改善すべき点が着取された。交通事故医療費については、相変らず、一部の地域の一部の医療機関において、相当高額の医療費請求がなされている状況について、調査を継続した。宮崎市では、健保基準の2倍程度の医療費請求が実状ではあるが、入院期間を長期化させるなどして、総額医療費は依然として高いという状況がみられる。関西では、一部の悪質な医療機関を除いても、診療技術基準は高く、静岡県でも1点当り、健保基準の2〜3倍ということも少なくない。医療費の基準策定のため、各地で医師会と支払側との間で話し合いが行われており、これらの動向についても調査した。なお、画一的な基準づくりに対しては、独占禁止法との関連で問題がありはしないかという疑念について、独占禁止法の専門家を招いて、この点の問題解明を行った。第3年度は、これまで仕残したことをほぼおえたが、医療制度,保険制度など複雑な問題がからむだけに、明確な括論は早急には出せないが、これまでの研究方針で、ほゞ問題点の検討をおえたので、収集した資料,調査結果に基いて、まとめの作業に入っている。
