急性期帯状疱疹痛の治療成績の解析では帯状疱疹そのものの重症度を考慮にいれることが重要である。基礎疾患がない患者では水痘・帯状疱疹ウイルス(以下,VZV)に対する最高の補体結合抗体価により帯状疱疹重症度を客観的に定義することが可能であり,最高の抗体価と急性期帯状疱疹痛の治療期間の対数値とには正の直線相関が認められるので,帯状疱疹の発症急性期から神経ブロック療法を開始した症例で,補体結合抗体法とenzymeーlinked immunosorbent assay(ELISA)により抗体価を経時的に測定し,帯状疱疹の重症度および最高の抗体価と治療期間との関係について検討した。〔対象と方法〕皮疹発症10日以内に急性期帯状疱疹痛に対する治療を開始し,基礎疾患を有していない31症例(軽症群6例,中等症群17例,重症群8例)について検討した。急性期帯状疱疹痛は疼痛を確実に除去できる局所麻酔薬を用いた神経ブロック療法を全例に疼痛が消失するまで実施し,抗ウイルス薬のアシクロビル(500mg/日,5日間)を併用した。VZVに対する抗体価は初診時,その後は一週毎に採血し,ELISAと補体結合法で測定し,最高のELISA値と補体結合抗体価を求め,治療期間は対数処理を行い相関関係を求めた。〔結果〕重症度別の年齢ならびに初診までの期間に3群間で有意の違いはなかった。治療期間は軽症群10^<1.387>±^<0.293>(平均24.4日),中等症群10^<1.748>±0.291温(平均55.9日),重症群10^<1.952>±^<0.275>(平均89.6日)であり,中等症群と重症群は軽症群に比して有意に(P<0.05)長期の治療を必要としていた。また,最高の補体結合抗体価と治療期間とには1ogY=0.1319X+0.7918と有意の(r=0.62,P<0.01)相関が認められた。しかし,今回の解析では最高の補体結合抗体価と最高のELISA値には相関を認めることはできず,また最高のELISA値と治療期間とにも有意の相関を認めることはできなかった。今後,症例を重ねる必要があると思われる。

The treatment of herpes zoster pain in the acute phase is characterized by analysis of the severity of herpes zoster. At the beginning of the VZV antibody test, the blood samples were detected immediately after the test, the ELISA antibody assay was used to determine the antibody, the highest ELISA antibody was combined with the antibody assay, and the number of the patients in the treatment period was calculated by correlation analysis.
