私どもは基礎疾患がない帯状疱疹患者では水痘ー帯状疱疹ウイルスに対する最高の補体結合抗体価が帯状疱疹重症度の客観的な指標となり,最高の補体結合抗体価と急性期帯状疱疹痛が消失するまでに必要とした交感神経ブロック療法の期間とには有意の相関があることを明らかにした.今回は補体結合抗体価よりも抗体価をより連続的に定義することが可能であるenzymeーlinked immunosorbent assay(ELISA)により水痘・帯状疱疹ウイルスに対するIgGおよびIgM抗体価を経時的に測定し,最高のIgG抗体価ならびにIgM抗体産生の有無と交感神経ブロック療法の期間との関係について検討した.【対象と方法】皮疹発症10日以内に急性期帯状疱疹痛に対して交感神経ブロック療法を開始し,基礎疾患を有していなかった56例(軽症9例,中等症32例,重症15例)について検討した.交感神経ブロック以外の治療法としてアシクロビルまたはビダラビン(500mg/日または600mg/日,5日間)を併用した.水痘・帯状疱疹ウイルス対する抗体価は初診時,その後は1週毎に採血し,ELISAで測定した.抗体価と治療期間との関係は治療期間の対数処理を行い,相関関係を求めた.【結果】重症度別に年齢ならびに初診までの期間に3群間で有意の違いは認めなかった.治療期間は軽症群では10^<1.427>±^<0.0091>(平均26.7日),中等症群では10^<1.758>±^<0.061>(平均57.3日),重症群では10^<1.920>±^<0.089>(平均83.2日)であり中等症は軽症に比べ(P<0.05),重症は中等症に比べ(P<0.05)長期の治療を必要としていた.ELISAでの最高のIgG抗体価と治療期間には相関は認められなかった(r=0.071).IgM抗体が検出されたのは軽症例で5例(56%),中等症例で15例(47%),重症例で11例(73%)と重症例での頻度は高くなっていたが,有意の違いではなかった.IgM抗体産生が認められた症例での治療期間は長い傾向であったが有意の違いではなかった(10^<1.785>±^<0.067>vs10^<1.703>±^<0.071>).
