「重症心身障害児」(以下,重症児と略す)とは重度の知的障害と重度の肢体不自由が重複した人たちである。全国の重症児数は約36,000名と推計されており,そのうち2/3の約24,000名が居宅で家族によってケアされている。本研究は,重症児施設と居宅や近隣の通園・通所センターを通信回線で接続し,在宅ケアを支援するITシステムを開発し,その有用性を実証することを目指した。本年度はまず始めに,在宅ケア支援のニーズを明らかにするために,在宅重症児と家族の協力を得て生活実態とIT支援に関するアンケート調査を行った。在宅重症児の主たる介護者は母親であり,母子の年齢差は概ね20〜40歳であった。介護者の年齢分布のピークは50歳代にあり,近い将来親の高齢化が進行する。そこでは在宅ケアが困難となり,施設入所へ移行するケースが増えることが予測された。在宅ケアから施設へスムーズに移行させるうえで,在宅の段階からITを利用して医療・福祉情報を遠隔収集し,データベース化していくこともシステム開発での重要な要件である。ITを利用した支援内容については<医療関連>の要望が大きく,続いて<生活関連>,<医療・福祉制度関連>が希望された。特に遠隔地では医療支援の要望が切実であった。一方,都市部では市内に重症児施設があることから医療支援は遠隔地ほどではなく,生活関連や医療・福祉制度関連など,より良いケアや生活の改善を望んでいるようであった。本調査研究から,IT支援システムには以下の4つの情報機能が必要であることが示唆され,こうした機能を備えた機器システムの開発に着手した:(1)音声情報:医療福祉に関する相談;(2)画像情報:顔色,褥瘡等の診察,リハビリ指導;(3)バイタル情報:脈拍数,呼吸数,血中酸素飽和度,血圧等;(4)データベース:健康,医療情報の経時的記録。

Special remote medical support is needed On the other hand, the Ministry of Urban Affairs has set up facilities for critical care in the city. Medical support has been provided to remote areas. Life is related to medical care and welfare system. Good health care and improvement of life are expected. In this study,IT support was initiated with the following four information functions:(1) audio information: medical well-being related conversations;(2) image information: color, bedsore, etc., diagnosis, and guidance;(3) digital information: pulse count, respiratory count, blood acid saturation, blood pressure, etc.; and (4) information: health, medical information, and time records.
