本研究の目的は、第1子を不妊治療により妊娠・出産した女性が第2子を妊娠するための不妊治療を選択する意思決定過程と、意思決定に影響する要因を明らかにすることである。さらに、第1子を不妊治療により妊娠・出産後の生殖の目標設定及び第2子以降のための不妊治療に関する意思決定過程を支えるための効果的な看護介入プログラムを作成することである。今年度は、第1子を不妊治療により妊娠・出産後1ヶ月以内の女性6名および第1子を不妊治療により妊娠・出産後、第2子のための不妊治療を開始して1年以内の女性15名を対象に半構成的面接を行った。対象者の平均年齢は33.7歳(範囲30〜42歳)、第1子のための不妊治療期間は平均2年2ヶ月(範囲7ヶ月〜6年0ヶ月)、一般不妊治療4名、高度生殖医療17名だった。第2子のための不妊治療を開始した16名の子どもの平均年齢は2歳1ヶ月(範囲10ヶ月〜3歳0ヶ月)だった。面接の結果から、意思決定過程には第1子の不妊治療経験を踏まえた不妊である状況の自己認識、自己や家族の心身社会的状況や能力などの状況認識、生殖や第2子以降のための不妊治療の目標設定、目標を達成するための選択肢の検討および計画の立案、計画の評価・修正が抽出された。しかし、多くは第1子のための不妊治療の悲嘆過程から回復しておらず、自己認識が不確実であった。さらに、目標設定や選択肢の検討および計画の立案に夫のかかわりが希薄であった。また、目標設定や選択肢の検討が十分なされておらず、第1子の育児と不妊治療の両立に困難をきたしていた。以上から、第1子を出産後、第1子のための不妊治療の悲嘆過程を促進させるかかわりと夫婦を対象にした生殖の目標設定、選択肢の検討および立案を含めた家族計画指導が必要であることが示唆された。今後、以上の結果を公表し、作成した看護介入プログラムを用いて、第1子を不妊治療により妊娠・出産した10組の夫婦を対象として看護介入を行い、介入プログラムの評価および修正を行う予定である。
