近年、医療は伝統医学(東洋医学)を再認識し、伝統医学の有用性を検討しはじめている。しかし、伝統医学の診察法が五感による主観的情報を主としていることから、客観的な方法による検討が課題となっている。そこで、伝統医学の診察法を客観的に検証するために、東洋医学の診察と生理学的検査を経時的に観察し、両者の関連を調査した。本年度は伝統医学の診断支援システムの検討と、東洋医学の診察法と生理学的検査(舌表面の血液循環動態、心拍、呼吸など)との関連性を調査し、東洋医学の診察法の有用性を検討したので報告する。1)伝統医学の診断支援システムの検討東洋医学の診察法の中で客観化に適した舌診に着目し、ファジィ理論を用いて、診察者の診断情報の処理過程により近い舌診断支援システムの開発を試みた。調査対象は本研究に対して同意が得られた患者ボランティア80例であった。舌診断支援システムの正診率を検証した結果、診察者の診断結果と支援システムの診断結果は7割以上の一致を認めた。伝統医学の診断支援システムは診察者における診断ロジックをシステムに取り込むことで、診察者の診断過程の一端を明らかにすることができ、診察者の判定に近い診断支援システムを構築することができた。このことは、伝統医学の診断支援システムを開発する上で、重要な参考事例となった。2)東洋医学の診察と生理学的検査との関連性成人ボランティア7名を対象に、東洋医学の診察法(舌診、問診(体調の状態))と生理学的検査(舌表面の組織酸素化血液量、心拍数、呼吸数、脈波)を行い、それらの関連性と6週間の経時的変化を調査した。その結果、交感神経を興奮させる電気刺激や音刺激により、舌表面の組織血液量が一過性に減少する傾向にあることが判った。また、体調に変化がある時では舌色に変化を認め、舌血液量が増加する傾向を示したことから、舌色や舌血液量が体調と何らかの関係にあるものと思われた。なお、舌血液量の変化と自律神経機能との関連性の詳細については現在検討中である。
