頭頚部扁平上皮癌に対するマトリックスメタロプロテアーゼ阻害剤の抗腫瘍効果

项目摘要

本研究はMMP阻害剤投与による腫瘍増殖抑制効果(腫瘍径、腫瘍重量、転移数)、Apoptosisへの影響を検索し、腫瘍細胞に抗CD34,Ki67抗体による免疫組織化学染色を行い、腫瘍増殖能、血管新生に対する抑制効果を検索した。また、CDDP,5-FU、TXTとの併用による相乗効果を検討した。また、放射線照射にともなう腫瘍増殖抑制効果も検討した。MMP阻害剤における頭頚部扁平上皮癌に対する抗腫瘍効果、他の抗癌剤併用による相乗効果を検討する。頭頚部扁平上皮癌において、MMPの発現と転移、予後との関係を示した論文は希少であり、また、MMP阻害剤と他の抗癌剤を併用した報告は皆無である。他の抗癌剤併用により副作用を軽減し、効果を高める治療法を検討するのが、本研究の最も大きな特色である。また、実際、臨床の現場にて、どのようにMMP阻害剤を使用すれば最も効果的か検索した。抗癌剤の投与にも様々な経路、方法がある。高齢化を迎える現在、患者のQOLを考慮し、外来経口維持化学療法として本剤を用いて、また他の抗癌剤や、治療法と併用することにより、副作用を軽減し、より高い効果が得られると期待される。具体的な臨床での投与方法についても検討した。研究を遂行するにあたり、横浜市立大学医学部耳鼻咽喉科学教室研究室を中心に実験を施行した。当研究室は、今までも、佃守教授を中心に頭頸部腫瘍の化学療法、遺伝子治療等、に関する研究を幅広く行っている。また、病理学第二教室に病理学的検索を施行する際に御指導していただき、また、神奈川県立がんセンター研究科、神奈川歯科大学生化学教室にも具体的な実験の技術指導をして頂いた。
