本研究は脳卒中の発症直後から在宅生活へのシームレスな移行を目指し、看護師が実施できる患者の活動量の増加と参加拡大のための支援方法の開発を行う研究であった。脳卒中は介護や療養を要する患者が多く、特に要介護度の高い患者の原因疾患の一つである。脳卒中患者が自宅に戻れない原因の一つとして、脳卒中による麻痺や意識障害に加えて、急性期から活動低下による廃用症候群が出現している場合が多い。脳卒中患者は、この廃用症候群の筋肉量の低下や可動域の縮小などによって、回復期病院や療養型の病院に転院となった後も、活動低下が改善できないことができず「寝たきり」になっている患者も多い。また、この「寝たきり」の状況のために、在宅に帰れないという状況になっていた。そこで、本研究では、脳卒中発症直後の急性期病院在院中から、在宅での生活にシームレスな移行できるように、看護師が行う日常生活援助に着目して、脳卒中発症直後からのICF（国際生活機能分類）のモデルに基づき看護師が活動・参加を促す、筋力低下や可動域の低下の予防などのプログラム開発する予定であった。令和4年度の予定としては、在宅へ移行してからの追跡調査を行う予定であったが、2020年度からのCOVID-19による病院への立ち入り制限などに伴い、患者を対象とした研究調査を行うことは困難であった。令和5年度は最終年度として、まとめを行うとともに、患者の活動量増加に関連した、看護師が使用可能な評価方法の開発をめざしたい。
