:「研究目的」喉頭摘出者の心理的適応の構造とソーシャルサポート及びQOLの影響を明らかにすることである。「研究方法」対象は、東京A会の喉頭摘出者会会員で平成17年度に会員登録されている1445名全員、九州B会の平成19年度の喉頭摘出者会会員383名に郵送調査法にて行った。調査内容は、喉頭摘出者用心理的適応尺度(NAS-J-L)、インフォーマルサポート尺度(MOS)、フォーマルサポート尺度(HPSQ-25)、QOL尺度(SF-36)、年齢、性別、職業、術後経過年数、会話手段について調査した。倫理的配慮に関しては、調査は任意であることを説明し、同意が得られた場合にのみ調査票を送付してもらう手続きをとった。分析方法は、共分散構造分析にて検討した。「結果」喉頭摘出者の心理的適応構造を共分散構造分析にて検討したところ、東京及び九州いずれも、3層構造モデル(GFI=.979, AGFI=.946、GFI=.971, AGFI=.926 )が最も適合度が高かった。潜在変数"自分が行動主体である認識"(観測変数:ローカルオブコントロール、自己効力感)が高まると、"障害の受容"(受容、態度)が高まり、"内面的な自己価値"(不安・うつ、自尊感情)が高まるという心理的適応構造が示唆された。さらに、ソーシャルサポートであるインフォーマル・フォーマルサポートの変数を投入して、共分散構造分析にて検討したところ、いずれも、潜在変数"自分が行動主体である認識"に影響していた。基本的属性としては、平均年齢70.6歳(東京)、71.7歳(九州)であった。性別比は9:1の割合で男性が多かった。術後経過年数は、東京では術後1年以上、九州では1年未満からを対象とし、20年以上の対象もみられた。 失声のため退職となった人は17.4%(東京)、14.1%(九州)を占めた。 「考察」自分が何か行動をおこそうとする感覚が高まることが重要で、自分の価値を見出すのは障害受容が高まった後という結果と考えられる。本研究結果の構造の意味することは、時間の流れを待ってただ機会が来ることを待つのではなく、自分が何もできない人間ではなく何か行動を起こすことができるのだという感覚を高め、まずは行動してみるという意識を持てることが、心理的適応を高める第1歩と考えられる。そこから成功体験を積み重ねることで障害を受容できる。障害受容が高まれば、自分自身の人としての価値を見出していける可能性を示唆している。"自分が行動主体である認識"が高まるようなサポートが重要であることが示唆された。この潜在因子は自分の意識の持ち方によって行動が決まるという感覚が高まることで、自己効力感及びリハビリテーション意欲を向上させていく因子である。自ら何かしようという感覚をもてるようなサポートが必要であることが考えられる。
