平成21年度は、看護実践および看護学教育の中に、その国の社会・文化的背景がどのように影響しているのかを明らかにするために、海外での労働経験をもつ看護者への聞き取り調査や、看護実践場面の観察、看護教育関係者との討議、教科書の内容の分析を行った。海外での労働経験のあるタイの看護師が看護実践場面で抱えた困難は、言葉の問題、移住先(西洋)の国の保健医療制度の違いによる困惑であったが、一番大きな困難は、看護師のもつ東洋的な価値観、信念、生活習慣等、文化的背景の相違であった。看護実践は、身体接触を伴い、また、相手の価値観・信念・文化を尊重し関わる職であるため、看護者の本来持つ価値観や、育った国の文化・宗教が、時には看護をする上でのジレンマになる。しかし、タイでは、他国への看護労働力移動を積極的に行っていないため、海外で働くことを想定した看護学教育はほとんど確立していない。一方、多くの看護師を主に米国に送り出しているフィリピンでは、看護学教育の中に、米国や英国での労働を前提とした教育カリキュラムが組み込まれており、さらに永住のニーズも高いことから、生活スタイルや文化に関する教育も行なわれている。インドネシアから日本への看護師受け入れは、平成20年から始まったが、来日した208人の中には、事前に日本語や日本の生活習慣を学んだ者は数名であった。さらに、インドネシア人の宗教に基づく価値観、性別による身体接触の制限等があり、日本での看護実践においては、多くのジレンマが存在することが予想される。看護教育関係者との討議においては、これまで西洋主導であった看護理論から、東洋の文化にねざした理論・教育方法の開発の必要性について共通理解が得られ、今後、協働して研究を進めていくこととなった。
