项目摘要

本研究は、母乳育児の継続または終了に関する母親の意思決定葛藤への支援プログラムを開発し、母乳育児中の母親を対象にした無作為化比較試験で意思決定葛藤の軽減への効果について検証し、母乳育児の継続や終了における支援のエビデンスを創出することを目的とした研究である。今年度は研究計画に基づき、母乳育児の継続または終了に関する母親の意思決定葛藤への支援プログラムの開発および資材の作成を行った。また、下記について関連学会で発表を行った。1）周産期の女性の意思決定葛藤への意思決定ガイドを用いた支援の効果について、国内外の介入研究17件の文献レビューを行った。レビューの結果、意思決定ガイドのテーマは、帝王切開術後妊娠の分娩様式7件、出生前診断6件、骨盤位の外回転術1件、無痛分娩1件などであった。媒体には冊子、ワークシート、音声ガイド、カード、ウェブサイトが用いられていた。介入効果に関して、意思決定葛藤の軽減に加えて、知識の向上、決定への満足度の向上が示された研究もあったが、一方で対照群との有意差が認められなかった研究もあった。介入による有害事象はいずれの研究においてもみられなかった。2）1歳6ヵ月健康診査を受診した子どもの母親158名を対象とした実態調査より、母乳育児の継続または終了に関する意思決定葛藤が高い母親では、ヘルスリテラシー高得点群の者は18.8％、中得点群39.5％、低得点群44.0％（p＝0.054）であった。交絡因子調整後の多変量解析の結果、意思決定葛藤が高群となるオッズ比は、ヘルスリテラシー高得点群と比較して、中得点群では2.98（95％CI 1.08-8.19, p＝0.034）、低得点群では3.48（95％CI 1.21-10.01, p＝0.021）であり、母乳育児の継続または終了に関する意思決定葛藤とヘルスリテラシーとの関連が明らかとなった。
