本研究全体の目的は、慢性疾患高齢者のフォーマルサービス（医療サービス、介護サービス）とインフォーマルサービスの利用実態や関連要因を明らかにし、在宅慢性疾患高齢者の状態像を考慮した生活構築のためのマネジメント指針の開発を行うことである。2022年度は、在宅慢性疾患高齢者のフォーマルサービスとして、医療サービス・介護サービスの利用実態の解明を目的に、A県の国民健康保険及び後期高齢者保険加入者の国保データベースおよび介護総合データベースのデータを用いて、在宅慢性疾患高齢者の状態像および状態像別の医療・介護サービス利用のパターンや影響要因を分析することを計画していた。実施した内容は以下である。まず、A県のレセプトデータ全体について入力方法を確認しデータの特徴を把握した。医療・介護サービスの利用やレセプトデータ分析に関する国内外の文献レビューを行った。また、在宅医療の実践家へのヒアリングを行い、慢性疾患高齢者の状態像を把握するためのレセプト項目や分析の枠組みを検討した。これらをもとに、最初の段階として、心不全高齢者に焦点をあて、分析対象となるデータの抽出および分析計画を立案した。抽出されたデータについて、記述統計やクロス集計を行いデータのクリーニングを実施した。今後は、状態像を示す様々な条件別に対象者を分類し、訪問看護の利用状況や他の医療・介護サービス利用との関連、転帰などについて分析をすすめる計画である。
