This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This study is designed to determine whether acid reflux (GERD) contributes to asthma and whether treatment with a proton-pump inhibitor (PPI) will reduce asthma exacerbations. This will be conducted as a multicenter, randomized, double-masked, parallel-group clinical trial, with a placebo control. Of 400 patients to be recruited nationally, 22 are expected to be studied here. The primary outcome measure is asthma exacerbation rate during treatment. Positive endpoints would include lower rate of asthma exacerbations, fewer asthma symptoms, better quality of life, better lung function, less health care use, and improved airway reactivity compared to those treated with a placebo. Secondary outcomes include measurable lung functions related to peak flow. The PPI is esomeprazole (Nexium). This is potentially of great benefit to symptomatic asthmatics who would benefit from a simple medical intervention (proton pump inhibitor).
