项目摘要

This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. Participants will be followed from 3 weeks before quitting through one year after the quit day. Participants will receive nicotine transdermal patch treatment (21mg/24hr.). 270 participants will be randomly assigned to receive a facility-based, supervised exercise intervention, and 270 participants will be assigned to a home-based structured exercise program. One-half of the participants assigned to the facility-based exercise sessions and one-half of those in the home-based condition will be assigned to receive additional cognitive-behavioral counseling aimed at increasing exercise adherence. The remaining 270 participants will not receive exercise adherence counseling, only brief exercise instruction. Every participant will undergo a graded exercise test, medical screening, complete psychosocial surveys, a blood draw, and DEXA scan for body composition during her first precessation visit. Each individual whose screening shows no contraindications to the use of nicotine patch, exercise testing and training, and who completes exercise testing and baseline questionnaires, will be randomly assigned to one of the four treatment conditions. Additional exclusions include active severe psychiatric illness and drug dependence. The exercise treatments will begin 3 weeks prior to the quit day and continue until 12 weeks after the quit day. All 540 participants will have 2 precessation visits, at 3 weeks and 1 week before the quit day, and will have regular postcessation follow-ups at 1,2,4,8,12,16,26,39, and 52 weeks after quitting. Follow-up visits will include questions regarding cessation progress, withdrawal symptoms and mood assessments, and reassessment of baseline characteristics such as commitment to cessation and biochemical verification of self-reported smoking status.
