Cranberry juice has been widely used in the prevention and treatment of urinary tract infections (UTI) as alternative therapy in spite of limited evidence of its efficacy. Concurrent use of cranberry juice during UTI antibiotic therapy is very common, especially for those who experience recurrent UTI infections. However up to date, little information is available regarding drug interactions with cranberry products. Thus identifying potentially adverse drug interactions between beta-lactam antibiotics and cranberry juice are of paramount importance given its great public health potential as a non-antibiotic alternative in UTIs prophylaxis and treatment. Several recent studies have shown that fruit juices may affect activities of membrane transporters which are important in the absorption and elimination of antibiotics. In light of clinical reports of drug-fruit juice interactions, we propose that cranberry juice ingredients or its byproducts arising from systemic metabolism may interfere with carrier-mediated intestinal absorption and/or renal excretion of beta-lactam antibiotics when it is co-administered with the antibiotics.
