This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The institution listed is for the Center, which is not necessarily the institution for the investigator. This study has two arms, the aim in study A is to determine whether ADPKD subjects with preserved renal function (GFR >60 ml/min/1.73m2) and high-normal blood pressure or hypertension, ACE-I/ARB combination therapy and low blood pressure vs ¿-blocker and usual blood pressure reduces the percent increase in kidney volume over time. The aim in study B is to determine whether ADPKD subjects with moderate renal insufficiency (GFR 30-60 ml/min/1.73m2), ACE-I/ARB combination therapy as compared to ACE-I, with additional antihypertensive agents at standard blood pressure control (?130/85 mm Hg), increases the time to doubling of serum creatinine, ESRD or death.
