This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This study has two arms, the aim in study A is to determine whether ADPKD subjects with preserved renal function (GFR >60 ml/min/1.73m2) and high-normal blood pressure or hypertension, ACE-I/ARB combination therapy and low blood pressure vs ¿-blocker and usual blood pressure reduces the percent increase in kidney volume over time. The aim in study B is to determine whether ADPKD subjects with moderate renal insufficiency (GFR 30-60 ml/min/1.73m2), ACE-I/ARB combination therapy as compared to ACE-I, with additional antihypertensive agents at standard blood pressure control (?130/85 mm Hg), increases the time to doubling of serum creatinine, ESRD or death.
