项目摘要

本研究は、精神科病院で働く看護師が倫理的な課題に遭遇した時、倫理的課題について察知し、困難な状況にあってもその状況について看過するのではなく、状況を適切に評価判断し、建設的な問題の解決を図ろうとする倫理的レジリアンスの様相と、職場環境、提供するケアの質からのモデルを構築することを目的としている。本研究のキー概念である「倫理的レジリエンス」について、Rushton（2018）の捉える定義から、「逆境にもかかわらずに看護師個人の全体性、統一性を維持し、回復する個人の能力」と捉えた。研究計画の遂行の状況は、数年続いたコロナ禍での対応と教育業務の煩雑さ（臨地実習の中止・学内実習への対応、業務の増加、さらに研究協力者の退職等により研究班の入れ替わり等）により、研究の進行が大幅に遅れている状況がある。研究内容に関わる精神科医療システム全体の現状として、看護師の倫理レジリアンスを職場環境から捉えた時、精神障害者にも対応した地域包括ケアが進められていいく一方で、特に昨今のマスコミ等でも報道されているように、精神科病院でのコロナ患者の対応とケアの課題、看護職者による虐待、全国的な身体拘束件数の増加、未だ適応されている精神科特例によるスタッフ人数の欠如など、安全で治療的な環境を提供するための医療システム全体の課題も多く指摘されている。このような日本の精神科医療の課題と、ポストコロナ禍で各精神科医療施設の抱える問題点や課題を加味し、看護者が倫理的な課題とそれに対応することに、どのようにケアの質を捉えるかについて海外文献を中心に未だ模索している状況である。早急にインタビュー調査を開始していく予定。
