本研究の目的はアクションリサーチによる保健師のアセスメント能力向上を導く看護過程の構築である。事例検討への介入は2018～2020年度の3年間で終了し、2021～2022年度は介入プロセスの企画評価を行った。都道府県型保健所の研修企画担当保健師6名のインタビューから課題認識と研修企画意図について質的分析を行い、学会に2題発表した。1)研修企画担当保健師の課題認識として、新任期保健師は保健師の役割イメージに曖昧さがあり、支援の視点が家族全体に広がりづらく、対象者の暮らしに戸惑いを感じ、家庭訪問の限られた場面の中で深い情報収集が難しく、行動変容を促す判断や介入に踏み込めないと捉えていた。組織の課題として家庭訪問の減少傾向から個別支援の積み重ねが難しく、先輩保健師が後輩の育成方法に確信を持てないこと等から職場全体で育ちあう体制づくりの必要性が示唆された。2)研修企画担当保健師は、研修対象の新任期保健師の支援者としてのアセスメント力の自己評価を起点に事例検討の構成を検討するなどアセスメント力を反映した企画を練っていた。事例検討は多様な事例から家族の生活ぶりを知り支援の展開がつかめるよう、生活と健康を関連させる思考を磨く問いかけを行うとともに支援のモチベーションを高める工夫を取り入れていた。新任期保健師と先輩保健師が個別支援における看護過程を共有し互いに学び合える事例検討が必要である。3)Ａ地区で行った新任期・プリセプター・管理期の保健師が参加し保健師経験年数別グループワークを用いた事例検討場面を分析し、論文を作成した。事例検討15事例のうち、母子事例6例と高齢者事例5例をそれぞれ集積して分析し、アセスメントの視点の特徴を検討した。アクションリサーチの結果をもとに、個別支援におけるアセスメント力向上を目指した保健師の看護過程指導手引書を作成中であり、2023年度に完成を目指す。

The results of nursing care are in the process of being developed, and individual support efforts are directed upward. The nursing process guidance manual for health care professionals is being developed and completed by 2023.
