研究目的:病院と処方箋応需薬局との連携を強化し、従来のお薬手帳に加えて検査値や主治医の意見、処方箋応需薬局からの情報を記載することで、患者が治療方針や現状を把握し、セルフマネジメント能力を向上させることを目的とした。研究方法:金沢市立病院の退院支援ワーキンググループで、薬物治療連絡日記に対する意見を得て素案を作成した。本研究の遂行を倫理委員会に申請し承認を得て、比較的理解力がある退院予定の患者に研究の主旨を説明し、薬物治療連絡日記について意見を求め改定した。特に工夫した点として、処方内容が退院時と同じ場合には、「前回Do」とのみ記載するかわりに、処方内容がいつでも閲覧可能なように表紙を長くして折り返して、しおり代わりに使えるようにした。再入院する患者も多いことから、再入院後の退院に対応するため、患者基本情報を上書きするためのシールも作成した。基本情報には、主治医の治療方針、入院中の治療内容、退院後にフォローすべき検査値を記載した。調剤日には、薬局からの意見、主治医の回答欄を設けて薬物治療連絡日記を完成し、日記の効果を確認するアンケート作成を実施した。研究成果:入院中に内服薬を自己管理し、薬剤師から薬剤管理指導を受けていた患者のアドビアランスは高く、薬物治療連絡日記に検査値や治療方針が記載されることで、退院後も治療経過を自分で確認できる点がこれまでのお薬手帳と比較して優れていた。特に、飲み忘れた余剰薬を主治医に伝えられることから、薬を無駄にすることがなく、コンプライアンス確認にも役立った。調剤薬局では、処方解析をすることなく正確な情報を得ることができる点、主治医に連絡したい事柄を伝えられる点が評価される。しかし、薬物治療連絡日記を適切に運用するためには、院内の薬剤師の支援が必須であり、.薬剤師外来などの整備が強く望まれる。アンケート結果の集計は学会等で発表予定である。
