本年は、精神科医療における継続的質改善活動の方法論の開発を試みた、精神科病院退院患者満足度調査、および総合病院精神科機能調査を実施した。精神科病院患者満足度調査は、現在分析中を終了し、国内外の医学専門誌へ投稿中であるが調査概要は次の通りである。調査対象は31の精神科病院の1997年8月10日から9月10日までに退院した全患者471例のうち、緊急転院などで調査不可能な48例、老年痴呆と精神遅滞47例、調査拒否の12例を除いた364例である。調査の結果326の質問票を回収した(回収率89.6%)。調査項目は、入院治療についての全体的満足度、スタッフの対応についての評価、入院および入院中の治療についての説明、および入院生活の快適さから構成される。総合病院精神科における医療機能調査については、国際学会で発表し、論文原稿は雑誌「総合病院精神医学」に受理されている。ここではその概要を示す。他科からの紹介患者の特徴を分析した。対象は、4病院での精神科における特定日の558名の入院患者と515名の外来患者である。受療経緯ごとに、患者特性、治療期間、機能の全般的評価を分析した。入院患者の15.9%、外来患者の10.3%が他科からの紹介患者であった。受療経緯では、紹介患者には器質性精神障害や精神作用物質使用による精神・行動障害の割合は多かったが、気分障害の割合は低かった。他科からの紹介による入院患者は、その他の患者と比較して、一般病棟への医療保護入院であり、精神科入院回数が少なく、機能レベルが低かった。他科からの紹介による通院患者は、通院期間が短く、合併症があった。本結果は、受療経緯によって患者特性が異なるために、それぞれに適切な精神科医療を提供する必要があることを示している。
