项目摘要

高知全県の精神科医療機関をベースとしたネットワークの形成、精神科医療の現状の調査と分析、および医療の質を高めるための提言を行うことを目的に研究を開始した。当初研究テーマとして精神科医療の受療行動等を例に検討し、ついで医療機関が保有するデータベースの活用も検討した。1.高知県の協力を得て気分(感情)障害の1ヶ月間の初診患者の動向を調査し、年間の自殺者は1人で初診患者の自殺率は予測された数字に極めて近い等の結果を得た。2.電子データベースの活用の例として、大学病院の25年以上にわたる電子データベースからフルボキサミンが検査値に与える影響を検討した。対象者は57人で、投与日、検査値等をエクセルファイルで処理したところ約7時間を要した。また同じ課題をSQL作成を用いると処理時間は1時間程度であった。さらにC言語を用いた場合は、このような処理が完全に自動化できるということであった(未実施)。3.データベース活用に関する精神科医療機関の現状と期待を見るためのアンケート調査を施行したところ(17病院から回答、回答率74%)、(1)レセコンは16病院で7種のソフトを使用、(2)処方・検査が電子化されているのは10病院、(3)電子カルテのレベルは1が5病院、2が1病院、4が1病院、(4)診療支援システムの利用は4病院、(5)電子カルテの2次利用への関心があるのは9病院、という結果を得た。4.今後様々な形でのデータの活用や複数の医療機関からのデータの結合のためには元データのデータベースからのデータ抽出、データ変換、データ編集と分析、グラフなどの表示といったプロセスを自動化することが求められ、そのような自動化が実現できれば所望の分析結果がリアルタイムで得られることが判明した。今後関心のある病院が連携すれば、これまでにない大規模なデータの活用と質の高い医療の展開が可能であるが、患者の匿名化などの倫理的配慮が重要課題である。
