これまで申請者は、インターロイキン1(IL-1α、IL-1β)およびインターロイキン1レセプターアンタゴニスト(IL-1ra)の歯周炎患者の歯肉溝滲出液中の動態について調べたところ歯周炎の重症度に密接に関連していることを報告した。そこで、これらIL-1およびIL-1ra量が重症度のみならず臨床的な病態の違いを反映するか否かについて、急速進行性歯周炎患者の歯肉溝滲出液中のIL-1およびIL-1raを測定し成人性歯周炎患者のそれと比較検討した。その結果、歯肉溝滲出液中のIL-1α、IL-1β量は、IL-1αが急速進行性歯周炎患者で高い傾向を示したものの両者に有意な差は、認められなかった。また、歯肉溝滲出液中のIL-1ra量は、成人性歯周炎患者に比べ急速進行性歯周炎患者において有意に低い値を示した。IL-1活性度においては、成人性歯周炎患者に比べ急速進行性歯周炎患者のほうが有意に高い値を示した。今回の結果から特に急速進行性歯周炎患者の歯肉溝滲出液中のIL-1ra量が成人性歯周炎患者より少なく、その結果IL-1活性度が、急速進行性歯周炎患者で高かったことは、IL-1raが急速な歯周組織破壊に密接に関与していることを示している。また、歯肉溝滲出液中のIL-1を測定するこたは、炎症の程度を把握するだけでなく病態の違いを判別する上でも非常に意義深いことが示唆された。

The applicant therefore reports on the dynamics of interdental exudate in periodontitis patients with IL-1 alpha and IL-1 beta, and on the relationship between interdental exudate and severity of periodontitis. IL-1 and IL-1ra levels were measured in crevicular exudates from patients with rapidly progressive periodontitis and compared with those from adult patients with periodontitis. The results showed that IL-1α and IL-1β levels in dental crevicular exudate were higher in patients with rapid progressive periodontitis than in patients with chronic periodontitis. IL-1ra levels in sulcus exudate were significantly lower in adult patients than in patients with rapidly progressive periodontitis. IL-1 activity was significantly higher in adult periodontitis than in rapidly progressive periodontitis. The results showed that IL-1ra levels in sulcus exudates from patients with rapid progressive periodontitis were lower than those from adult periodontitis, IL-1 activity was higher in patients with rapid progressive periodontitis, and IL-1ra levels were higher in patients with rapid progressive periodontitis. The determination of IL-1 in the exudate of the tooth groove is of great significance to the determination of the degree of inflammation and the identification of pathological changes.
