项目摘要

本研究の目的は1980年代以降の日本とフランスにおける医療保険改革の展開の異動を説明することである。とくに新自由主義的な政策アイディアが日仏において異なる仕方で追求されてきたことを、両国における政策アクターの制度的配置の差異に着目して解明することを目指した。研究計画に従い本年度は新自由主義的な医療保険改革はいかにして生じるのか(あるいは生じないのか)という観点から、80年代以降の日仏の医療保険改革の事例分析を行った。そのために当該テーマに関する研究書・論文及び政府審議会の報告書などの一次資料を検討した。また、渡仏して資料収集および現地の研究者との意見交換を行った。日本では90年代後半以降厚生(労働)省が経済財政諮問会議などの新たなアクターの台頭に直面し、医療の自由化を目指す政策アイディアが頻繁にアジェンダに載るようになった。同じ時期のフランスでは厚生官僚が政策への強い影響力を維持し、国家の役割の強化を目指す改革を推進してきた。そこで日仏における医療保険政策の主たる形成者である厚生官僚の政策形成上の影響力の差異に関する比較研究に取り組んだ。両国の厚生官僚の採用・昇進・政策アイディア・政策形成過程における影響力の推移に着目し、両国のコントラストを浮き彫りにした。他方、医療政治のもう一方の主役とも言うべき医師に関して、医師組織の独占性・集権性・政策形成過程への統合度に着目して、これらが相対的に高位にある国(日本・ドイツ)においては低位にある国(フランス)に比べで医療費抑制政策が成立しやすいという仮説の検証に取り組んだ。以上の研究を通じて様々な国家・非国家アクターの政策アイディアと制度上の位置が整理されたため、それらと医療保険改革の展開との関運に関する知見の総括に取り組んでいる。また、政治動向に応じて両国で今後採用される可能性のある政策シナリオを複数提示する作業にも着手した。
