本研究は、要介護の主要原因疾患である痴呆の発症要因を、地域コホート研究(nested case-control study)により分析し、痴呆の予防・介護予防につながる要因の検出を目的とする。発症要因としては、生活習慣(喫煙、アルコール等)等や痴呆の危険因子である可能性のある血清中のホモシステイン、高感度C反応性蛋白(hs-CRP)について分析する。対象は、茨城県K町(人口1.7万人)の1984年から1999年の循環器健診受診者(年間約3,500名)の中から、2000年4月から2006年9月までに介護保険認定を受けたあるいは今後受けると予想される65歳以上住民男女約660人である。介護保険認定データ(主治医意見書、認定調査票)を連結可能匿名化する。介護認定された痴呆者(痴呆の日常生活自立度がII度以上)をEndpointとして、その時点で介護認定されていない生存者を対照として、健診受診者の中から、性、年齢(±2歳)、健診受診年を1:2でマッチさせて無作為に選び出す。そして、過去の健診所見を痴呆と対照との間で比較する。また、-80℃で凍結保存している血清を用いてホモシステイン、hs-CRPの濃度を測定し、痴呆と対照との間で比較する。介護保険認定者約450人中、痴呆患者206人について、対照412人と合わせて解析を行った。その結果、喫煙に関する多変量調整オッズ比は、全痴呆で2.3(1.1-4.7)、脳卒中既往を有する痴呆で2.6(0.8-8.2)、脳卒中既往のない痴呆で2.2(0.8-5.7)であった。また、45年間以上の長期喫煙者で痴呆のオッズ比が2.3(1.0-5.4)であった。アルコール摂取、血清ホモシステイン、hs-CRPに関しては、痴呆との関連は認められず、hs-CRPは痴呆患者でむしろ低い傾向を示した。日本人の痴呆の発症に長期間の喫煙が関与している可能性が示された。
