本研究は、臓器移植患者の体験からスピリチュアリティを引き出し、スピリチュアリティを支える看護ケアについて検討することである。臓器移植患者に非構造的面接法により印象に残っている出来事や体験について自由に語ってもらった。面接回数は1人2〜4回、1回の面接時間は約1〜2時間であった。データ分析は、人間存在を時間性を基盤にして解釈する立場に立つ解釈学的方法論を用いて分析した。解釈の妥当性は分析結果を研究参加者に確認してもらった。ここでは2事例について報告する。事例1(腎機能獲得と喪失を体験し再透析になった女性):患者は喪失体験による実存的な苦しみに苦悩しているとき、大きな木に出会った。その大木に生命力を感じ、木も自分も同じ「命」を共にする存在であることを知覚し、木が木のために生きているように「自分は自分のために生きればいい」ことに気づいた。その木によってその人のスピリチュアリティは解き放たれ、木が「命」を共にする存在として見いだされた。事例2(心臓移植後にクローン病を再発した男性):患者は心臓移植で与えられた命によって生きる力が喚起された。カテーテル検査、クローン再発、失職などが繰り返されるなかで、ボランティア活動、結婚、仕事探し、就職と前向きに行動していた。生きることを励ます妻や友人や親など他者とのつながりが生きる力を与えていた。他者が「生きる」という目的に向かって共に進む存在として見いだされた。それらの結果から、スピリチュアリティを支えるには、患者と何かを共にする存在が重要であることが明らかになった。臨床の場で病む人間のスピリチュアリティを支える看護ケアには、「何かを共にする存在」としての看護者の存在が必要であると思われる。そのために、看護者は患者の求めている何かを共有できるように、まずは患者の語りをじっくり聴きことであり、そこからケアが始まることが示された。
