项目摘要

1.在宅健康管理プログラム携帯テレビ電話による健康相談、尿糖および血圧の自己測定、生活習慣記録、栄養・食事調査等を用いた通信型在宅健康管理プログラムを開発した。一般住民を対象に本プログラムを実施し、各種身体計測・血液生化学所見、健康関連生活習慣に関して、プログラム前後の変化を評価した。結果として、ヘモグロビンAlc、血清総コレステロール等の検査値の改善、健康意識の向上、生活習慣の改善が認められた。本プログラムは、対象者の健康ニーズに応じた多様なサービスを行うことが可能であり、携帯電話を用いた通信型在宅健康管理プログラムの有効性と実用性が確認された。2.在宅介護支援プログラム大学、対象者家庭、保健センターを拠点とし、双方向動画通信末端(テレビ電話)、バイタルサイン測定機器より在宅テレケアシステムを構築した。従来型の訪問サービスに加えて在宅テレケアを実施したテレケア群と通常の在宅ケアサービスのみを実施した対照群について、身体機能、運動機能、精神機能、日常生活動作(ADL)自立、コミュニケーション自立、社会的認知自立、活動能力、主観的健康観について、Functional Independence Measure (FIM)、Katz ADL自立指標、老研式活動能力指標、PGCモラールスケールを使って、サービス実施前後の変化を比較した。テレケア群は、ADL自立度、コミュニケーション自立度、社会生活能力自立度について統計学的に有意な改善がみられ、改善度は対照群に比較して有意に大きかった。タイムスタディの結果、ケアの提供に要した専門職の従事時間は両群で有意な差は見られなかったが、テレケアを含むケア回数は在宅ケア群で有意に高かった。在宅テレケアの利用は、専門職の従事時間を増やすことなく、ケア回数を増やし、高齢者の自立度をより高めることが実証された。
