これまで、成人の歯周疾患において炎症との関連が示されているIL1-α、IL1-β、IL1-2、IL1-6、TNF-αについて、歯肉溝滲出液(GCF)中の量と炎症との関連をダウン症児(D群)と健常児(N群)で比較することを目的して本研究を行った。被験者は、上顎中切歯が萌出しているダウン症児(平均年齢10歳7月)および健常児(平均年齢10歳8月)である。GCFの採取部位は、上顎前歯部唇側の歯肉溝で、被験歯の歯面に付着している歯垢を短針、小綿球で取り除き、コットンロールで簡易防湿し、エアで乾燥後、濾紙(ペリオペ-パ-^<(R)>)を歯肉溝に挿入、30秒間、または60秒間静置してGCFを採取し、すみやかに濾紙をスクリューキャップ付マイクロチューブに移し、密閉した。GCFの採取量は、採取の前後に濾紙とスクリューキャップ付マイクロチューブの重量を測定し求めた。GCF中のサイトカインの定量は、ELISA法を用いた測定用キット(Quantikine^<TM>, R&D systems)を使用した。被験歯のポケットの深さは、すべて2mm以下でD群:1.6±0.7mm、N群:1.8±0.3mm、Loe and SilnessのGIはD群:1.3±0.8mm、N群:1.1±0.7であり、ProbingによるBleedingは両群とも75%に認められた。30秒間当たりのGCF採取量は、D群:0.24±0.15μl、N群:0.38±0.25μlであり、有意にD群が少なかった。IL1-2、IL1-6、TNF-αは、どの試料においても検出されなかった。IL1-α、IL1-βは全試料から検出され、IL1-αは、D群:32.6±30.6pg/30sec、N群: 12.9±6.2pg/30sec、IL1-βは、D群:3.8±1.4pg/30sec、N群:5.9±2.3pg/30secであった。これらサイトカインとGIとの関連は両群とも認められなかった。また、ダウン症児では、GCF量が少ない、濾紙の挿入によっても容易に出血するなど、GCFを検体とする上での困難さが多々あった。

This study was conducted to compare the relationship between inflammation in periodontal disease (PD) and normal control group (N) in adults, including IL-1-α, IL-1-β, IL-1 - 2, IL-1 - 6, TNF-α and GCF. The upper jaw incisors develop symptoms (average age: 10 years and 7 months) and healthy children (average age: 10 years and 8 months). GCF is taken from the maxillary anterior teeth, the teeth on the labial side of the tooth surface are coated with short needles, small cotton balls are removed, and the filter paper (optional-^<(R)>) is simply moisture proof. After drying, the filter paper is inserted into the tooth groove for 30 seconds, and the filter paper is allowed to stand for 60 seconds. GCF is taken, and the filter paper is removed and sealed. The amount of GCF taken before and after the filter paper was measured.
