本年度はまず、2002年4月から7月まで、昨年度から継続してきた、米国ミシガン州アナーバーの二つの在宅ホスピス・プログラムを対象とする包括的なフィールドワーク(継続的インタヴュー、会議の録音、訪問ケア随行、内部資料閲覧、スタッフ間コミュニケーションの記録等を含む、参与的観察)を実施した。帰国後は、フィールドから得たデータをまとめると共に、我が国のホスピス・ケアに関する情報を精力的に収集し、日米の実践に見られる差異の発見を通じた現代ホスピス運動の相対化を試みた。本年度の研究からは、文化人類学と我が国のホスピス・ケアの双方に対し、特筆すべき新しい視座を提供することができた。文化人類学に対しては、死の統制に関する新しい記述の様式を開発した。現代ホスピスは、その全人的なシステムの作動を通じ、巨大で、曖昧で、対処困難な恐怖の対象であった「大きな死」を、各々は小さく、具体的で、対処可能な「小さな死」へと分解するルーチンを生み出しつっある。本研究は、この作動の詳細な記述を通じ、具体的目標の設定/達成という手続きの連鎖へと死が分解される過程を捉えたのである。我が国のホスピス・ケアに対しては、これまでに培ってきた「ホスピス・ケアにおける教育」という視座をさらに拡充し、単なる教唆を越えた双方向的コミュニケーションとして教育を再定義すると共に、ホスピス・ケアにおける「教育モデル」(教育を契機として専門職-一般市民の知識・技術差を可能な限り縮めるアプローチ)と「向上モデル」(専門職がその知識・技術を可能な限り高め、資源を集約的に活用するアプローチ)の提案を通じて学際的議論の基盤を形成した。以上の成果は、学会等で発表すると共に、特に我が国のホスピス・ケアに関する考察を『米国ホスピスのすべて:訪問ケアの新しいアプローチ』(ミネルヴァ書房)にまとめ、上梓した。
