项目摘要

It is difficult to differentiate which precancerous lesions are going to malignant tumors. To differentiate the precancerous lesions which are going to malignant tumors from those which remain not to be malignant, we studied the expression pattern of oncogenes, c-fos and c-jun, immunohistochemically about various oral lesions : squamous cell carcinoma, leucoplakia, lichen planus, papilloma, etc.The clinical materials were fixed in 10% formalin, embedded in paraffin and cut in 5μ thickness by a microtome. After incubation with anti-c-Fos and c-Jun antibody, the sections were stained by rhodamine-conjugated IgG for c-Fos and by FITC-conjugated IgG for c-Jun at the same time, and observed by a fluorescent microscopy.Stainability of the epithelial lesions was divided into c-Fos(+)/c-Jun(+), c-Fos(+)/c-Jun(-), c-Fos(-)/c-Jun(+) and c-Fos(-)/c-Jun(-).c-Fos(+)/c-Jun(+) cells were observed beneath the keratinized layer and sporadically at the basal cell layer of the normal oral mucosa, leucoplakia and lichen planus lesions. C-Fos(+)/c-Jun(-) cells were detected in few cases of normal oral mucosa, but in the squamous cell carcinoma of infiltrative tendency, some parts of leucoplakia and lichen planus lesions with down growth tendency. C-Fos(-)/c-Jun(+) cells were observed scatteringly in the squamous cell carcinoma with frequent mitosis.From the above findings it may be suggested that the lesions of leucoplakia and lichen plamus with c-Fos(+)/c-Jun(-) cells have a tendency to change into malignancy.
很难区分哪些癌前病变是恶性肿瘤。为鉴别恶变的癌前病变和未恶变的癌前病变,我们用化学方法研究了口腔鳞状细胞癌、白斑、扁平苔藓、乳头状瘤等多种口腔病变中癌基因c-fos和c-jun的表达。用抗c-Fos和c-Jun抗体孵育后,分别用罗丹明标记的c-Fos和FITC标记的c-Jun免疫荧光染色,荧光显微镜下观察,将病变的可染性分为c-Fos(+)/c-Jun(+)、c-Fos(+)/c-Jun(-)、c-Fos(+)/c-Jun(-)、c-Fos(+)/c-Jun(-)、c-Fos(+)/c-Jun(-)和c-Jun(-)。c-Fos(-)/c-Jun(+)和c-Fos(-)/c-Jun(-)、c-Fos(+)/c-Jun(+)细胞在正常口腔粘膜、白斑和扁平苔藓皮损的角化层下方和基底细胞层中偶见。C-Fos(+)/c-Jun(-)细胞在正常口腔黏膜中仅少量表达,在浸润性鳞状细胞癌、部分白斑和扁平苔藓病变中呈向下生长趋势。C-Fos(-)/c-Jun(+)细胞在有丝分裂频繁的鳞状细胞癌中呈散在分布,提示c-Fos(+)/c-Jun(-)细胞阳性的白斑、苔藓病变有恶变倾向。
