项目摘要

本研究の目的は、重度の慢性疾患・精神障害・他の嗜癖との合併、高齢化などのハンディを同時にもっているゆえ、従来の「入院→通院・抗酒剤・セルフヘルプグループ参加」の医療を中心とした社会復帰モデルでは解決しえない多様な生活問題・課題を抱えているアルコール依存症の地域生活支援システムを開発し、未整備地域にシステムを導入するための方策を検討することである。そこで、入所・通所施設のリハビリテーションプログラムを利用している利用者の援助事例の質的分析手法によって地域生活ニーズを特定するとともに援助過程の可視化を試みた。その結果、入所施設・通所施設と形態の違いや個々の利用者の特性の違いはあるものの、利用者のニーズ・介入には以下の共通項が見いだされた。本人は、飲酒問題の解決、対人関係・健康・生活上の問題、意味ある環境との絆・関係の再構築・創造、地域への移行、就労など多様なニーズを抱えていた。援助者は、利用者を生活者とらえ、地域の一員として暮らしていくために本人の問題・課題への介入を行っていた。とくに本人との関係性が適合的なものになるようニーズに応じて家族、セルフヘルプ・グループ、地域、就労のための資源など地域の諸資源への働きかけによって、病気や問題を抱えつつも地域の中に受け入れられる関係性の創造が示された。また、地域社会・学校への働きかけ、フォーマル・インフォーマルなネットワークキングによって利用者のニーズを充足するとともに、地域のアルコール問題・生活課題へのニーズ充足に貢献していることが示唆された。さらに、断酒が継続し日常生活も安定しているものの、年齢・社会経験・身体状態などにより本人の力を社会に還元できるシステムの不備が浮き彫りになったが、障害程度区分認定に対象に相当しないという新たな問題も発生し、いかにシステムを作っていくかは今後の課題とする。
