本研究の目的は、ハイリスク新生児の母乳育児を成功に導くために母親の搾乳体験を分析し、母親の視点から看護援助を導き出すことである。平成17年度の計画はデータ収集および分析、研究の総括であった。対象はNICUに入院している新生児の母親20名で、この内10名は電動式搾乳器を使用し、10名は用手搾乳を行った。電話による半構成型の聞き取り調査を行い、調査内容は搾乳中の疲労や痛み体験、搾乳の手順や操作に関する体験、搾乳期間における印象的な体験等とした。分析は内容分析の手法による記述的分析を行った。用手搾乳、電動式搾乳使用の場合それぞれ、出生児の平均在胎週数は29.0週と28.6週、平均出生時体重は933gと958gであった。電動式搾乳器使用の場合、搾乳の手順や操作に困難さを感じた母親はいなかった。疲労や痛み、困難さをまったく感じなかった母親は、用手搾乳の場合1人、電動式搾乳器使用の場合5人であった。乳首や乳房の痛みの理由に誤った搾乳方法があった。電動式搾乳器使用の場合、高い圧の方が早く搾れるとの誤解や、圧の調整が可能なことを知らずに高圧で使用していた。搾乳体験は、用手搾乳の場合「搾乳を続けるべきか否かという葛藤」そして「搾乳を中止したことによる解放感」であった。電動式搾乳器使用の場合「安楽で快適な母乳分泌の維持」だった。電動式搾乳器使用の場合、多くの母親が痛みや疲労を感じず、快適に搾乳を行えていた。一方、用手搾乳の場合には痛みや疲労を感じながらの搾乳であったため、搾乳を続けることへの葛藤や、搾乳をやめたことによる解放感を感じたと考えられた。よって痛みや疲労の点から、電動式搾乳器は重要な資源であると考えられた。また、電動式搾乳器を誤った方法で使用し痛みを感じていた母親もおり、搾乳を快適に行うために適切な使用方法の指導ができるよう、医療者の十分な学習が必要であると考えられた。
