非定型抗精神病薬は定型抗精神病薬に比較し錐体外路症状等の副作用の発現率が低いため、現在統合失調症薬物治療の中心的役割を担っている。近年、アメリカ食品医薬局(FDA)は、高齢の認知症患者に非定型抗精神病薬を用いた場合に死亡率が上昇すると報告し、高齢の認知症患者に対する非定型抗精神病薬の使用を警告した。この注意喚起は、統合失調症の高齢者に対する非定型抗精神病薬使用を控え、代償的に定型抗精神病薬の使用を促す可能性がある。しかし、定型抗精神病薬は非定型抗精神病薬に比較し錐体外路症状をきたしやすいことから、高齢の統合失調症患者に抗コリン薬の併用が今後増えることが予想される。抗コリン薬は、アルツハイマー病の病理に関連するアミロイドβの沈着を促進するという報告があることから、我々は、高齢の統合失調症患者における抗コリン薬使用とアミロイドβの沈着の関連について免疫組織学的に調べた。前年度は、学内倫理委員会の申請許可、福島県立医科大学精神疾患死後脳バンクよりサンプルを収集し免疫組織染色を実施した。本年度は、前年度の実験結果を解析し、以下の結果が得られた。1.日本の統合失調症患者死後脳では、アミロイドβの沈着である老人斑の密度は増えていなかった。2.抗精神病薬の使用量と統合失調症患者の老人斑の密度との間に有意な相関を認めなかった。3.統合失調症発症年齢と老人斑の密度との間には負の相関を認め、統合失調症患者には何らかの老人斑形成を抑制する性質がある可能性が示唆された。4.他の認知障害を引き起こす原因タンパク質(リン酸化タウなど)の免疫染色条件を検討し決定した。5.本研究において、抗コリン薬使用とアミロイドβの沈着の関連性について、統計的な有意差は認めなかった。その理由として、抗精神病薬自体がアミロイドβの沈着を抑制する可能性があること、サンプルサイズが小さいことなどが考察された。
