広汎性発達障害(以下、PDD)を養育する家族の育児ストレスと家族機能, QOLの現状調査 ; PDD児(3-6Y)の母親149名への無記名質問紙調査を行った.調査内容は基本的属性、自閉症の子どもの行動特徴質問紙(行動特徴), PS-SF実用版, WHO-QOL26, 家族機能尺度(FAI)で、有効回答数は78部(52.3%)、児の平均年齢5.4歳±1.1(男児66, 女児12), 診断は自閉症54%、PDD 26%、HFA13%、Asperger 4%であった.児の行動特徴総点と母親のストレスに有意相関はないが、行動特徴の「限局行動」とPS-SFの「子どもの特徴に関するストレス」は正相関した(Spearman's ρ=0.54, P<0.01).「母親自身に関するストレス」は、QOL-26全領域と負に相関し(身体領域ρ=-0.63, 心理的領域ρ=-0.55, 社会的関係ρ=-0.55, 環境ρ=-0.61, P<0.01)、FAI「家族に対する評価」とも負に相関した(ρ=-0.66, P<0.01).PS-SF因子分析では6因子構造((1)孤立・自信のなさ(2)子の行動の困難さ(3)育児をめぐる夫との関係(4)子の好ましい行動の少なさ(5)育児による心身不調の自覚(6)子の不機嫌)で、子どもの行動特徴3側面が、PSI-SFの第4因子と、こだわり行動がPSI-SFの第2・4・5m因子と有意に関連した.家族の育児ライフスキル促進プログラム; Strength based approachを重視したHealthy Start Program研修(オレゴン州)で認定証を受けた.調査結果に基づき、母親の心身リフレッシュと癒しを重視した小人数制プログラム「すきっぷ・ママクラス」を考案し、平成20年12月より開始した.平成21年度末まで第5期生(延べ42名)に実施し、継続して介入研究を実施中である.データは現在解析中であるが、第2期生までの結果、<導入前><3 か月後><導入後>を比較すると、母親の育児ストレスPS-SF得点は有意に低下し、QOL・家族機能は上昇した.

A survey on the status of family parenting, family function, and QOL was conducted among 149 mothers of PDD children (3- 6Y). The survey included basic attributes, behavioral characteristics of autistic children, PS-SF practical version, WHO-QOL26, FAI, 78 responses (52.3%), average age of children 5.4 years ±1.1 years (66 boys, 12 girls), diagnosis of autistic children 54%, PDD 26%, HFA 13%, Asperger 4%, and so on. There was a positive correlation between the behavioral characteristics of children and maternal traits, and a positive correlation between the behavioral characteristics of children and maternal traits (Spearman's ρ=0.54, P<0.01). QOL-26: Negative correlation across all domains (Physical domain ρ=-0.63, psychological domain ρ=-0.55, social relationship ρ=-0.55, environmental ρ=-0.61, P<0.01), FAI "family related to the evaluation" and "negative correlation"(ρ=-0.66, P<0.01).PS-SF factor analysis was used to construct six factors: (1) isolation, self-confidence,(2) difficulty of child action,(3) relationship between children and parents,(4) good behavior of children,(5) psychosomatic disorder of children,(6) child's lack of mechanism,(3) behavior characteristics of children,(4) bottom, PSI-SF factor,(5) and (6). 2. 4. 5m factor of PSI-SF and intentional correlation. Family parenting promotion; Strength based approach; Healthy Start Program training (state); certification. The results of the survey showed that the mother's mental health was improved, and the number of children was increased. The investigation began in December 2010. The fifth student (42) at the end of the 21st year of Heisei The PS-SF score of the mother's child is intentionally lower than that of the QOL family function.
